I HEREBY ACKNOWLEDGE that I am over the age of eighteen (18) years, and that all of the information set forth in this credit statement is true, accurate and full and complete disclosure thereof. I am providing written consent under the Fair Credit Reporting Act for above client and its partners with whom I am matched to obtain a consumer credit report from a contracted credit bureau. I understand that I am submitting a prequalification request for credit, and am consenting to the use of my credit report information. I authorize any holder of this credit application or any person, firm or corporation requested to extend credit there under, (including any employee or agent of any of them) to communicate with my employer in order to verify my employment. I authorize any holder of the loan or Retail Installation Contract, the creditor thereof, or any Attorney, debt collector or collection agency to communicating any and all information concerning this application or debt to any credit reporting agency or other creditor. By providing my email address, I consent to receive electronic information such as monthly billing reminders, statements and collection notices. I also acknowledge that you and your partners may use all contact information provided to contact me regarding this application, loan offer, account status or future issues. You may utilize electronic, mobile, autodialed messages, SMS or traditional methods. I further acknowledge and agree, that I will notify the creditor or prospective creditor in writing of any change in my name, address or employment within a responsible time thereafter.

I expressly authorize the third party lender or program manager I am matched with to share among its affiliates, loan servicers, and bank partners any transaction history related to my financial products or services received or serviced through the third party lender for the purpose of evaluating me for credit. 

 

State Specific Disclosures

Notice to married California residents: California Civil Code §1812.30(j) requires we notify you that if you, the applicant, are married, you may apply for a separate account.

Notice to Married Wisconsin Residents: No provision of a marital property agreement, a unilateral statement under Wis Stat §766.59 or a court decree under Wis. Stat. 766.70 adversely affects the interest of the creditor unless the creditor, prior to the time the credit is granted, is furnished a copy of the agreement, statement or decree or has actual knowledge of the adverse provision when the obligation to the creditor is incurred.

For OH applicants: The Ohio laws against discrimination require that all creditors make credit equally available to all credit worthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio civil rights commission administers compliance with this law.

For CO applicants: Your loan application may be subject to an additional review using artificial intelligence (AI). This technology may help applicants with limited credit history by providing a more comprehensive evaluation. The AI tool analyzes the information you submitted including previously obtained credit, financial, or other personal data to assess your overall credit profile. This process supports fair and informed credit decisions and may improve your chances of approval.

Verification

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

I authorize the Social Security Administration (SSA) to verify and disclose to U.S. Bank through Early Warning Services, LLC. or Socure Inc., for the purpose of this application whether the Name, Social Security Number (SSN) and Date of Birth (DOB) I have submitted matches information in SSA records, including the basis for a no-match response. My consent is for a one-time validation within the next 90 days. By clicking the ‘Submit application’ button, I am signing the consent for SSA to disclose my SSN Verification to U.S. Bank / Early Warning. I agree that my electronic signature has the same legal meaning, validity and effect as my handwritten signature. 

 

Informed Consent

  • I agree to U.S. Bank and its third-party service providers conducting a soft credit check using the personal information I’ve provided, which is my own and accurate, and which will not impact my credit score.
  • I agree U.S. Bank and its third-party service providers may share my contact information and the disposition of this application with my provider and its service providers.
  • I agree to receiving all electronic documents and disclosures for my U.S. Bank Avvance application which requires a valid email address and internet access. To request free paper copies or update my contact information, I may call U.S. Bank at 800-210-2364.
  • I agree that if I am approved and as part of reviewing the terms of a U.S. Bank Avvance installment loan agreement, my FICO score and U.S. Bank Avvance Account Number will be used for display purposes in the United Credit environment.

Loan performance disclosures to merchant providers

In connection with the facilitation of consumer financing through our platform, we may share limited loan performance information, specifically whether a consumer’s loan resulted in a first payment default, with the originating merchant or service provider. This information is shared solely for the purpose of administering merchant funding programs, enforcing merchant repurchase obligations, and mitigating potential fraud or misuse of financing services.

This disclosure is limited to transactions in which the merchant was directly involved and is subject to confidentiality and non-reuse restrictions. We do not share full credit reports or other consumer financial data for unrelated purposes.

After a transaction, your private information (credit cards, social security numbers, financials, etc.) will be kept on file for more than 60 days in order to abide to state and federal financial laws.
